“Youth Program SENSE Comments in OSOS”, 2021 — programme-specific, and three years older than the SSAN guide.

NYSDOL publishes a shape for these: SSAN — Situation, Summary, Assessment, Next Steps. Barriers to employment go in the Assessment, and any warning given about continued Unemployment Insurance eligibility has to be documented there too. The Youth Program has its own variant, SENSE, which adds Sufficient Information and Employment-Related — programme-specific, and three years older. In OSOS a note takes two saves: one for the comment dialog and another for the whole customer record. Four guides say so, which suggests people lose notes to it.

The sample note above is adapted from the one NYSDOL prints in its own IEP guide. Read it as evidence rather than decoration: a complete employment plan — objective, labour-market rationale, provider, accommodations, next contact date — living in a free-text box, none of it reportable, none of it queryable. A modern build would capture those as fields and let the note carry what only a person can say.
